IMPLEMENTATION OF RATIONAL MEDICATION USE PROTOCOLS IN THE HOSPITAL PHARMACY

Hospital pharmacy has undergone significant restructuring, evolving from an essentially logistical unit to a strategic sector with clinical and care functions, aimed at promoting the safe and rational use of medicines. This transformation is supported by the National Pharmaceutical Care Policy and the regulation of the clinical pharmacist's role. The objective is to investigate the implementation of rational drug use protocols in the hospital pharmacy, analyzing its contribution to patient safety, treatment efficacy, and the qualification of pharmaceutical care, while identifying the main protocols, challenges in application, and impacts of the pharmacist's performance. The study was conducted through a literature review, analyzing recent research and data on irrational drug use, the historical evolution of hospital pharmacy, the pharmaceutical care cycle, and the clinical practice of pharmacists in different hospital contexts, such as ICUs, geriatrics, and neonatology. The review demonstrates that the clinical practice of the hospital pharmacist, based on protocols, is fundamental for identifying and resolving Drug-Related Problems (DRPs), reducing medication errors, adverse events, and hospital costs. Case studies evidence successful interventions that improved therapeutic adherence and clinical outcomes. However, challenges persist, such as a lack of standardization, inadequate infrastructure, and a shortage of specialized professionals.
