ADVERSE EVENT REPORTING AND PATIENT SAFETY IN PRIMARY HEALTH CARE: A STUDY IN A MUNICIPALITY IN PARANÁ

Patient safety is one of the fundamental pillars of quality healthcare. This study aimed to analyze adverse event reports in Primary Health Care in a municipality in the state of Paraná. This is a quantitative and descriptive documentary research, carried out from the analysis of 131 notification forms registered between May 2024 and February 2025. The results showed a predominance of errors related to medication and medication dispensing. A higher occurrence of events was observed in women, adults, and the elderly. Most events did not result in harm to the patient. It is concluded that notifications are fundamental tools for strengthening the patient safety culture, and it is necessary to invest in the qualification of records and in the continuous training of teams to improve the quality of care in Primary Health Care.
