PROJETO DE INTERVENÇÃO EM PACIENTES IDOSOS DIABÉTICOS E HIPERTENSOS DA ESTRATÉGIA SAÚDE DA FAMÍLIA JOAQUIM PEDROSA

Este estudo avaliou o uso e o acesso ao serviço envolvido na Atenção Primária à Saúde juntamente com o Programa Hiperdia para os pacientes portadores de Hipertensão Arterial e Diabetes na rede pública inserida dentro da Estratégia de Saúde da Família Joaquim Pedrosa. A Hipertensão Arterial Sistêmica (HAS) e a Diabetes Mellitus (DM) são problemas de saúde pública e são responsáveis por inúmeras complicações nos indivíduos acometidos por essas patologias, sendo que a prática de exercícios físicos associada a uma alimentação saudável é fundamental na prevenção destas doenças. A criação do programa HIPERDIA foi essencial para redução na prevalência da hipertensão e diabetes no Brasil. A atuação de uma equipe multidisciplinar na Estratégia de Saúde da Família, juntamente com o apoio da equipe do NASF é de extrema importância para que os pacientes obtenham resultados satisfatórios. A pressão alta é um dos principais fatores de risco para a ocorrência de acidente vascular encefálico, infarto agudo do miocárdio, aneurisma arterial e insuficiência renal e cardíaca. Na assistência integral à saúde, é necessário tratar o indivíduo como um todo e a Unidade de Saúde da Família se inserem neste contexto de prevenção, promoção e assistência, para evitar complicações, como o controle dos níveis pressóricos e glicemia capilar. O presente projeto visa qualificar o processo de trabalho na Estratégia de Saúde da Família (ESF) do PSF Joaquim Pedrosa, com proposta de capacitar a equipe sobre a classificação de risco para o acompanhamento adequado a cada usuário, vistas à promoção em saúde dos usuários portadores de HIPERDIA dessa localidade, como o objetivo de implantar oficinas de educação permanente, visando à prevenção, promoção e controle destas doenças que contribuirão efetivamente para a conquista de uma saúde mais estável em cada idoso, tornando-os sujeitos ativos e participativos, na sociedade da qual fazem parte.
