LEPROSY IN PRIMARY HEALTH CARE: CHALLENGES FOR EARLY DIAGNOSIS, CONTACT SURVEILLANCE AND DISABILITY PREVENTION IN THE BRAZILIAN UNIFIED HEALTH SYSTEM

Abstract

Leprosy remains a neglected tropical disease of major public health relevance in Brazil, particularly in territories affected by poverty, social inequalities, access barriers and weaknesses in Primary Health Care (PHC) organization. This integrative review analyzed evidence published between 2016 and 2026 on the challenges faced by PHC within the Brazilian Unified Health System regarding early diagnosis, active contact surveillance and prevention of leprosy-related physical disabilities. The review followed identification, screening, eligibility and inclusion stages, with reporting guided by PRISMA 2020. Original quantitative, qualitative, mixed-method, evaluative and intervention studies were considered, together with essential technical and normative documents. Findings indicate that delayed diagnosis is associated with limited clinical confidence, low diagnostic suspicion, stigma, territorial inequalities and insufficient integration between care and surveillance. Contact examination remains heterogeneous and frequently incomplete, restricting interruption of transmission. Disability prevention is weakened by failures in disability grading and documentation, limited self-care education and discontinuity of follow-up. Leprosy control therefore requires territorially responsive PHC, continuing professional education, systematic active case finding, care coordination, improved information quality and intersectoral actions addressing stigma and social vulnerability.
