O USO DE OPIOIDES NO MANEJO DA DOR NOS CUIDADOS PALIATIVOS ONCOLÓGICOS: BENEFÍCIOS E RISCOS

Introdução: Os opioides são substâncias com propriedades farmacológicas semelhantes aos opiáceos, derivados do ópio, extraído da Papaver somniferum. Esses fármacos têm uso clínico amplo, especialmente em cuidados paliativos oncológicos, onde o controle da dor representa um dos principais desafios. Objetivo: Discutir as implicações dos opioides no tratamento da dor em pacientes com câncer avançado, destacando seus benefícios e os riscos associados. Metodologia: Este estudo utilizou uma revisão integrativa de literatura, formulando a pergunta: "Qual é o impacto dos opioides no manejo da dor em cuidados paliativos oncológicos?". Foram realizadas buscas nas bases SCIELO, PUBMED e LILACS com descritores como “opioides”, “cuidados paliativos” e “manejo da dor”. Foram incluídos artigos de 2014 a 2024, em português, inglês e espanhol, que abordassem o uso de opioides em cuidados paliativos. Resultados: A busca resultou em 2.557 artigos, dos quais 103 foram selecionados após leitura do título e resumo, e 15 artigos foram incluídos após análise completa. Os principais achados incluem revisões sobre a classificação dos opioides, estudos sobre sua eficácia no controle da dor e discussões sobre os riscos de efeitos adversos, como depressão respiratória e tolerância. Discussão: O cuidado paliativo busca melhorar a qualidade de vida de pacientes e familiares frente a doenças graves, com foco no alívio da dor. Os opioides são eficazes para o controle da dor, mas apresentam efeitos colaterais e podem contribuir para a eutanásia indireta. Conclusão: Os opioides são essenciais nos cuidados paliativos, mas seu uso requer manejo cuidadoso para evitar efeitos adversos, mantendo o foco no alívio do sofrimento com uma abordagem ética.
