Child health care in the first two years of life is essential to ensuring healthy growth and development while preventing conditions that may compromise quality of life in the long term. This article analyzes data from the National Program for Access and Quality Improvement in Primary Care (PMAQ-AB), focusing on the 306 primary care teams evaluated in the state of Tocantins, Brazil. Results showed significant findings, such as 91.2% of teams conducting well-child visits and 97.4% actively searching for children with delayed vaccination schedules. However, gaps were identified, such as the lack of systematic records on family violence (39.9%) and child accidents (37.6%), highlighting the need for more robust actions in specific areas. The results reflect the importance of initiatives like PMAQ-AB in identifying best practices and addressing weaknesses within Brazil’s Unified Health System (SUS). The high coverage of critical practices, such as the use of the Child Health Record (96.4%) and monitoring of growth and development (89.5%), indicates alignment with national guidelines. However, challenges in record management, continuity of care, and outreach to vulnerable populations, particularly in rural areas, underscore the need for strengthened actions.
