Introduction: Intellectual disability is characterized by significant limitations in intellectual functioning and adaptive behavior, affecting conceptual, social and practical skills. Individuals with this condition often experience barriers to oral health care and present higher prevalence of oral diseases. Objective: To report the clinical management of a traumatic oral lesion in a pediatric patient with intellectual disability and epilepsy using antimicrobial photodynamic therapy.Case report: An 11-year-old male patient diagnosed with moderate intellectual disability and epilepsy was treated at the Special Care Dentistry Clinic of the Federal University of Rio Grande do Sul. The patient presented after a seizure-related fall with dental fracture and traumatic lip lesions. Treatment included dentin protection using glass ionomer cement and antimicrobial photodynamic therapy with 0.01% methylene blue associated with red laser irradiation. Conclusion: Antimicrobial photodynamic therapy proved to be an effective minimally invasive approach for the management of traumatic lesions in patients with special needs, contributing to tissue repair and improved clinical management.
