MANEJO DE LA INESTABILIDAD HEMODINÁMICA EN PACIENTES GRAVES SOMETIDOS A HEMODIÁLISIS

La hemodiálisis se indica comúnmente en pacientes críticos por diversas afecciones, incluyendo la lesión renal aguda (LRA). Durante la terapia de diálisis, pueden ocurrir episodios de inestabilidad hemodinámica significativa, en particular hipotensión arterial. Esta revisión integrativa tuvo como objetivo identificar y sintetizar las estrategias actuales para el manejo de la inestabilidad hemodinámica inducida por hemodiálisis en pacientes críticos. La búsqueda de artículos entre 2015 y 2025 se realizó en las bases de datos PubMed/MEDLINE, CINAHL, Cochrane Library, LILACS y BDENF. Se encontraron 105 estudios. Después de aplicar los criterios de inclusión y exclusión, se seleccionaron 5 artículos para la lectura completa. Los resultados mostraron que las estrategias más efectivas para prevenir y manejar la hipotensión intradialítica son la infusión intravenosa de albúmina y el uso de dializado frío. La modulación del sodio también puede ser efectiva cuando se combina con un perfil de UF ascendente-descendente. Se necesitan más estudios multicéntricos con muestras más grandes para guiar mejor la práctica clínica.
