DIAGNÓSTICO Y TRATAMIENTO DEL IMPÉTIGO NO AMPOLLOSO EN UN PACIENTE PEDIÁTRICO: REPORTE DE UN CASO CLÍNICO.

El impétigo es una infección cutánea bacteriana superficial causada por bacterias Gram-positivas, que afecta comúnmente a niños de 2 a 5 años, aunque puede ocurrir a cualquier edad. Clínicamente, el impétigo se clasifica en ampolloso y no ampolloso. El primero se caracteriza por la formación de ampollas grandes y frágiles. En el impétigo no ampolloso, existe la presencia de vesículas y pústulas que, tras su ruptura, dan lugar a costras de color miel. El caso clínico descrito reporta a un paciente de 12 años con antecedentes de psoriasis y dermatitis atópica, quien presentó lesiones vesiculares escamosas y dolorosas en el labio y la boca, sin un factor desencadenante, asociadas con síntomas sistémicos. Tras la persistencia de los síntomas y tras la evaluación de pruebas de laboratorio que indicaron una infección bacteriana, la patología fue diagnosticada como impétigo no ampolloso. El tratamiento se llevó a cabo con el uso de antibióticos, terapia fotodinámica y medidas de higiene, resultando en una mejoría significativa y el alta hospitalaria después de cinco días. Sin embargo, se observan algunas discrepancias en la literatura, destacando la gravedad del caso debido a la presencia de síntomas dolorosos, síntomas sistémicos y el grupo de edad más afectado. El caso subraya la importancia del diagnóstico diferencial entre el impétigo y otras infecciones cutáneas, así como del manejo terapéutico adecuado para evitar complicaciones y promover la recuperación completa.
