EVIDÊNCIAS RECENTES SOBRE OS MECANISMOS IMUNOPATOGÊNICOS DA DERMATITE HERPETIFORME: UMA REVISÃO INTEGRATIVA

A dermatite herpetiforme é uma dermatose bolhosa autoimune crônica, reconhecida como manifestação cutânea da doença celíaca, caracterizada por lesões papulovesiculosas pruriginosas e depósitos granulares de imunoglobulina A (IgA) na derme papilar. Esta revisão integrativa teve como objetivo sintetizar evidências científicas recentes acerca da fisiopatologia da doença, com ênfase nos mecanismos imunológicos que conectam o eixo intestino–pele. A busca bibliográfica foi realizada na base PubMed por meio de estratégia estruturada utilizando o descritor “dermatitis herpetiformis” associado a termos relacionados à patogênese e imunopatogênese. A busca inicial identificou 378 estudos; após aplicação de filtros para publicações dos últimos cinco anos e disponibilidade de texto completo gratuito, 21 artigos permaneceram elegíveis para análise preliminar. Mediante critérios de inclusão e exclusão previamente definidos, oito estudos compuseram a amostra final, sendo analisados qualitativamente quanto aos mecanismos imunológicos, inflamatórios, moleculares e histopatológicos envolvidos na doença. Os achados demonstram que a exposição ao glúten desencadeia resposta imune adaptativa intestinal com produção de autoanticorpos IgA contra transglutaminases, especialmente a transglutaminase epidérmica (TG3), principal autoantígeno implicado na formação das lesões cutâneas. Os imunocomplexos TG3–IgA, formados predominantemente no intestino, depositam-se na junção dermoepidérmica, promovendo ativação do complemento e recrutamento de neutrófilos, cuja liberação de enzimas proteolíticas contribui para a clivagem da membrana basal e formação de bolhas subepidérmicas. Evidências adicionais indicam que, embora a sensibilização seja intestinal, a fase efetora cutânea apresenta perfil predominantemente neutrofílico e relativa escassez de células T locais reativas ao glúten. Conclui-se que a dermatite herpetiforme constitui modelo de autoimunidade mediada por IgA dependente da interação intestino–pele, embora persistam lacunas na compreensão da relação entre deposição de IgA e atividade clínica.
