PREVENTION OF CHRONIC DISEASES: COMMUNITY STRATEGIES IN PRIMARY CARE

Non-communicable chronic diseases represent one of the main contemporary challenges for health systems, especially in the face of population aging, changes in lifestyles, and the persistence of social inequalities that influence the processes of illness. In this context, Primary Health Care assumes a strategic role in organizing actions aimed at the prevention, monitoring, and control of these diseases, especially through interventions developed at the community level. This study aims to analyze the role of community strategies implemented in Primary Health Care in the prevention of non-communicable chronic diseases. This is a qualitative research, with a bibliographic design and a descriptive-analytical approach, developed from the analysis of scientific productions that discuss the prevention, monitoring, and management of chronic diseases in the context of primary care. The literature review showed that the territorial action of health teams, combined with the performance of educational activities, health promotion actions, and continuous monitoring of users, contributes significantly to the reduction of risk factors associated with chronic diseases. It was also observed that interdisciplinary action and active community participation strengthen prevention strategies and increase the effectiveness of actions developed in health services. However, challenges related to the structure of services, intersectoral articulation, and the continuity of interventions carried out in the territory persist. It is concluded that strengthening community strategies within the scope of Primary Health Care is a fundamental element for health promotion and the prevention of chronic diseases, and it is necessary to increase investments in public policies that favor integrated, participatory, and territorialized care practices.
