DETERMINANTES DE PERDA E ALTERAÇÃO DO FLUIDO SINOVIAL NA ARTICULAÇÃO TEMPOROMANDIBULAR: UMA REVISÃO SISTEMÁTICA DE BIOMARCADORES MOLECULARES

Resumo

Esta revisão sistemática identifica e analisa criticamente os fatores determinantes associados à perda, alteração ou desregulação da homeostase do líquido sinovial (LS) na articulação temporomandibular (ATM). Uma revisão sistemática rigorosa foi conduzida de acordo com as diretrizes PRISMA, com buscas nas bases de dados PubMed/MEDLINE, Europe PMC, LILACS e SciELO até 11 de janeiro de 2026. De 5.361 registros iniciais, 32 estudos foram incluídos para síntese qualitativa. As evidências coletivas destacaram consistentemente o papel fundamental dos mediadores inflamatórios nas alterações do LS. Níveis elevados de citocinas pró-inflamatórias (IL-6, IL-8, TNF-α) foram frequentemente relatados em pacientes com deslocamento interno e osteoartrite. O fator de crescimento endotelial vascular (VEGF) demonstrou correlação significativa com a degeneração articular, e o derrame articular detectado por ressonância magnética apresentou forte associação com o aumento das concentrações de biomarcadores inflamatórios. A proteína 1 relacionada a Dickkopf (DKK1) emergiu como um modulador crítico na patogênese da osteoartrite da ATM, com seus níveis diminuindo significativamente após intervenções terapêuticas com ácido hialurônico. A patogênese da perda e alteração do líquido sinovial na ATM é complexa e multifatorial, impulsionada principalmente por uma cascata de mediadores inflamatórios, degradação enzimática e estresse mecânico. Estratégias terapêuticas voltadas para a modulação dessas vias inflamatórias e a restauração da composição do líquido sinovial, como artrocentese e suplementação com ácido hialurônico, demonstram benefícios clínicos promissores.
