INTEGRATION OF PALLIATIVE CARE IN THE CARE OF ELDERLY PEOPLE AFTER ACUTE MYOCARDIAL INFARCTION IN THE HEALTH SYSTEM: A SCOPING REVIEW

Abstract

Objective: To map evidence on the integration of palliative care for elderly patients after acute myocardial infarction (AMI) in the hospital setting. Methods: This is a scoping review, where the search was conducted in the following databases: PubMed, Scopus, Embase, Web of Science, LILACS, and grey literature. Original studies (quantitative, qualitative, or mixed) with elderly patients (≥60 years) diagnosed with AMI that addressed palliative care were included. Results: 568 articles were found, of which 10 remained after the selection stages. An increase in the temporal use of palliative care was observed, but it is still in a low proportion compared to hospitalizations for AMI. The indication was associated with greater clinical severity (cardiogenic shock, cardiac arrest, do-not-resuscitate orders). A strong association was found between palliative care and end-of-life decisions, with late and poorly systematized integration. Gaps were identified regarding early integration and patient-centered outcomes (quality of life, symptom control). Conclusion: The integration of palliative care for post-MI elderly patients remains limited and delayed. Integrated care models, professional training, and increased scientific production are necessary to promote early incorporation and improved quality of care.
