N-acetylcysteine is a compound known for its antioxidant properties. Considering its effects on conditions such as endometriosis, polycystic ovary syndrome, infertility, this study aimed to review the applicability of NAC in women's health. To conduct this research, original articles from the literature were examined using the Scientific Electronic Library Online (SciELO), National Library of Medicine (PubMed), and Virtual Health Library (VHL) databases, spanning the period from 2014 to 2024. A total of 28 articles were selected for review. NAC operates through several mechanisms that contribute to its efficacy, the primary one described in the literature being its role as a glutathione precursor. However, other mechanisms have been evaluated, such as its potential to enhance cell viability by inhibiting apoptosis or programmed cell death. Furthermore, it increases the activity of antioxidant enzymes, such as catalase and superoxide dismutase, which results in cellular protection against oxidative damage. The reviewed articles reported that NAC can alleviate pain and reduce the size of endometrial lesions, in addition to potentially enhancing fertility. In PCOS, NAC has demonstrated efficacy in restoring ovulation rates and improving oocyte quality, thereby contributing to higher pregnancy rates. Evidence suggests that NAC may exert a protective role against oxidative stress and inflammation, conditions frequently associated with disorders of the female reproductive system. The described mechanisms and the obtained results render the supplement promising and underscore the necessity of a nutritionist's involvement in its prescription. Its utilization, coupled with a balanced diet and lifestyle, is capable of promoting better outcomes in female reproductive health. Therefore, NAC is described as a favorable intervention, but its clinical application requires further scientific investigation.
