Dengue is a major public health concern, especially in tropical regions such as Brazil, where climatic, environmental, and socioeconomic factors favor vector proliferation. In the state of Piauí, the disease shows an oscillatory pattern with recurrent outbreaks, making it essential to analyze its epidemiological profile. This is a descriptive, retrospective epidemiological study with a quantitative approach, based on secondary data from the Notifiable Diseases Information System (SINAN), available through DATASUS. All probable dengue cases reported in Piauí from January 2020 to December 2025 were included. Variables analyzed included year of notification, sex, age group, race/ethnicity, hospitalization, and case outcomes. Data were organized and analyzed descriptively. A total of 69,792 probable dengue cases were reported. There was an increase between 2020 and 2021, followed by a peak in 2022. A decrease occurred in 2023, followed by a new increase in 2024 and a decline in 2025, demonstrating a cyclical pattern. Most cases occurred among individuals aged 20–59 years, females, and those self-identified as mixed race. Most cases evolved to recovery. Teresina accounted for the majority of cases. Dengue remains a major public health issue in Piauí. Strengthening epidemiological surveillance, vector control, and improving environmental and social conditions are essential to reduce disease burden.
