Diabetes mellitus (DM) is a chronic condition that impairs glucose metabolism, leading to persistent hyperglycemia and promoting the development of severe complications, which affect quality of life and increase the need for hospitalizations, particularly among the elderly population. This study aimed to characterize and compare the profile of hospitalizations of elderly individuals due to DM in the state of Maranhão between 2019 and 2025. It is an ecological, retrospective, descriptive, and quantitative study conducted using secondary data obtained from the Hospital Information System of SUS (SIH/SUS), available on DATASUS. During the analyzed period, 39,026 hospitalizations of elderly people due to diabetes mellitus were recorded in Maranhão, with the highest absolute number of cases in the North macro-region (21,906 hospitalizations). Nevertheless, the South macro-region had the highest proportional hospitalization rates (707.50 per 100,000 elderly). A predominance of emergency hospitalizations was observed, accounting for 92.0% of the cases (n=35,909). Regarding age group, although the highest absolute number of hospitalizations occurred among elderly people aged 60 to 69 years (n=17,450), the highest hospitalization rates were observed among individuals aged 80 years or older, especially in the Southern macro-region (7,047.71 per 100,000 elderly people). There was also a predominance of females among hospitalizations (52.6%; n=20,543). It is concluded that hospitalizations due to DM in elderly people in Maranhão presented high magnitude and significant regional inequalities, highlighting the need to strengthen Primary Health Care (PHC), especially in the continuous monitoring of the elderly population and in the prevention of complications related to DM.
